Intraocular pressure asymmetry means there is a noticeable difference in the fluid pressure inside the two eyes. The pressure inside each eye is normally similar, so when one eye measures higher or lower than the other it can be a clue that something is off. This condition does not describe a single disease but rather a finding that can point to many possible causes, such as early or unilateral glaucoma, inflammation, trauma, or even measurement error. Often there are no symptoms, so people may not know there is a difference until an eye exam measures it. Eye doctors use instruments called tonometers to measure this pressure and may repeat tests or use different methods to confirm that the difference is real. Because higher pressure in one eye increases the risk of damage to the optic nerve, asymmetry is taken seriously and usually prompts further testing like imaging of the optic nerve and visual field checks. Sometimes the difference is temporary and resolves on its own, while other times it signals a chronic problem that needs treatment to protect vision. Treatment depends on the cause and can range from topical eye drops that lower pressure, to laser procedures, to surgery in more advanced cases. Regular follow-up is important because changes can happen slowly, and early treatment can prevent permanent vision loss.
